When the user selects "Yes" with detail or "Detailed no" for a data item that has additional or hidden information, the additional information is immediately opened.  In this example, expanded information for general problems is opened when the users selects "Yes" with detail.  The expanded information is indicated by a arrows on the left side of the window.  This functionality speeds data entry by allowing the user to only view detailed data fields required for each patient. In this example, general problems include significant weight change, fever/chills, nights sweats, etc.

Each section can be as general or as detailed as required by the provider, practice or hospital.  In a teaching environment, the subscreens can be very specific to remind new physicians-in-training of symptoms to consider in making a diagnosis.  In a less structured practice, the subscreens can be very minimal and may only include a notes field.

The [SECTION NEGATIVE] button is another timesaving feature that can be used to select the "No" or "Negative" choice for all the expanded data items in that section.  Instead of selecting each choice individually, the user can select the [SECTION NEGATIVE] button.  Then, if needed, they can change only those items that are positive.

Many fields have a "Notes" button at the end of their list of choices.  Any additional data that is not fully addressed on the form and its subscreens can be detailed in the free text field.

The Chief Complaint field and the History of Present Illness field are both examples of "free text fields" or "word processing fields".  While it is desirable to code as much information as possible, these two fields can have such varied responses that coding would not be practical.  As an alternative, medEntry provides the "Phrase Builder" to minimize unnecessary typing as displayed on the Chief Complaint free text window above.

After the user enters the narrative into a free text field and selects "commit changes", the text is copied back into the form and displayed.
